Yizkor Book of Remembrance
If you would like to have the name of a loved one listed in Chevrei Tzedek’s Yizkor Book, please submit the names of your loved ones and make your payment online or download this form. If you download the form, mail the completed form to Nathan Rosen at his home address, listed on the form or email the completed form to [email protected] .
Please write the names clearly, exactly as you want them to appear in the book.
NOTE: Relationship(s) of the loved ones named to the person(s) submitting names will NOT be included in these listings.

All Yizkor book submissions MUST be made by Friday August 25, 2023 Absolutely NO exceptions.
Recommended donations are $18 each for the first three names and $10 each for additional names are requested.
Names listed together, i.e., Joe and Jane Smith, are counted as TWO names.
Donations can be made here or by mailing a check payable to Chevrei Tzedek (with your completed form). If paying by check, please write "Yizkor Book" on the memo line.
Special Commemorations
We are offering the opportunity for you to remember your loved ones with a unique commemoration.
You may submit text, a drawing, a photo, or a combination, but it must be something
that will reproduce well in black and white.
Photos and drawings can be emailed to [email protected] and submitted as a PDF
or mailed to Nathan Rosen at the address on the form.
Please keep your submissions proportional to the size you have selected
and make sure your commemoration will be legible in the size you have chosen.
We may put a border around your submission.
The suggested donations are as follows:
✡ $36 for half page (approximately 3 1⁄4" high by 4")
✡ $72 for full page approximately 7 1⁄4" high by 4" wide)
✡ $100 for inside back cover (approximately 7 1⁄4" high by 4" wide)
