Yizkor Book of Remembrance

Sorry, we are no longer accepting submissions to the Yizkor book for this year. 

If you would like to have the name of a loved one listed in Chevrei Tzedek’s Yizkor Book, please complete and submit this online form OR download this form. If you download the form, mail the completed form to Nathan Rosen at his home address, listed on the form.

 If you are keeping your entry the same as last year's, please email
yizkor@CTbalt.org to use the same information. You don't have to complete the form, but you DO need to submit payment.

All Yizkor book submissions MUST be made by Sunday August 15, 2021.  Absolutely NO exceptions.

Donations of $18 each for the first three names and $10 each for additional names are requested. Names listed together, i.e., Joe and Jane Smith, are counted as TWO names.  Print names clearly, exactly as you want them to appear in the book.
NOTE: Relationship(s) of the loved ones named to the person(s) submitting names will NOT be included in these listings.

Donations can be made here, by using the DONATE link, or by mailing a check payable to Chevrei Tzedek (with your completed form). If paying by check, please write "Yizkor Book" on the memo line.

We also offer the opportunity for you to remember your loved ones with a unique commemoration.  You may submit text, a drawing, or a photo, but it must be something that will reproduce well in black and white.  While you may mention that your commemoration is to honor the memory of one or two people who are of special significance to you, it will not be considered a substitution for the regular listings.

If you wish to provide only text and have it typeset, we can certainly do that; if you have a preference for a particular font, please mention it.  Photos and drawings can be emailed and submitted as a PDF. Hebrew characters can be treated as a picture as well; if you wish us to typeset it, please provide the EXACT Hebrew spelling you wish to appear. We would appreciate an extra donation of $18 for the extra effort.  We can also put a border around your submission.  

The suggested donations for the special commemorations only are as follows:

$36 for half page (approximately 3 ¼" high by 4")
$72 for full page approximately 7 ¼" high by 4" wide)
$100 for inside back cover (approximately 7 ¼" high by 4" wide, only one available)
 
Please make sure your commemoration will be legible in the size you have chosen.
